Our office accepts traditional Medicare as well as many commercial insurances including:

Please contact our office or check with your benefit provider handbook if your insurance company is not listed above. We will bill your insurance company for your office visit and will accept your co-pay or deductible at the time of service.
New Patient Forms
In preparation of your new patient appointment with Dr. L.A. Torres, Jr., please print out the forms below and make sure to have them filled out prior to your appointment. Also, please be sure to bring your insurance information and photo identification.
If for any reason you are unable to print out the forms, please call our office and we will have a set mailed to you.
